Your proactive approach will help resolve issues, improve short and long-term outcomes, and identify opportunities to enhance our members' wellbeing.Key Responsibilities• Develop and implement a proactive strategy to resolve issues and improve member outcomes• Conduct comprehensive assessments of members' needs and benefit plan eligibility using clinical tools and data/information review• Apply clinical judgment to integrate strategies that reduce risk factors and barriers, addressing complex health and social determinants that impact care planning and issue resolution• Evaluate data from multiple sources to address all conditions, including co-morbid and multiple diagnoses that affect functionality• Review prior cases to address potential impact on current case management and eligibility• Assess members' level of work capacity and related limitations/abilities• Utilize a comprehensive approach to evaluate the need for referrals to clinical resources to support functionality• Collaborate with managers and others to overcome barriers in meeting goals and objectives, presenting cases at multidisciplinary case meetings to support overall case management• Use case management processes in compliance with regulatory and organizational policies and procedures• Employ strong interviewing skills to ensure maximum member engagement and understanding of their health status and needsEssential QualificationsTo succeed in this role, you'll need:• A current and unrestricted RN licensure in your state of residence• Willingness and ability to obtain additional state licenses upon hire (paid for by the organization)• 3+ years of acute hospital clinical experience as an RN (general medical, post-surgical, ICU experience)Preferred Qualifications• 3+ years of case management experience• Compact RN licensure• Certified Case Manager (CCM) certification• Proficiency in a variety of Microsoft Office applications, including PowerPoint, Excel, and Word• Strong telephonic communication skillsAbout CVS HealthAt CVS Health, we're dedicated to delivering innovative, human-driven healthcare services that make a positive impact on people's lives.
